The reason is because the form could be rather long and arduous to complete if you are not aware of what is required. My suggestion is keeping it simple and using the recommended PMI  format. Do not use an essay format as this could throw off the reviewer! Use this:  IN - initiating PL  -  planning EX -  executing MC -  monitoring and controlling CL -  closing Project Description I - Developed the project charter with the project sponsor and identified stakeholders with the core team. Held kick-off meetings with stakeholders and presented the project charter for buy-in at higher levels. Created and maintained a stakeholder register. PL - Developed the project management plan components with the team. Built the schedule, WBS, cost baseline, quality plans, communication plans, risk plans, procurement plans and stakeholder engagement plans. EX - Conducted team meetings and directed the project work while managing the developers on the project. Ensured all issues were identified and tracked. Engaged risk owners and action owners to execute planned responses. Inspired, motivated and mentored team members. Maintained an impediment log and was responsible to impediment resolution with stakeholders. MC - Created and maintained regulatory reports for the project. Created and distributed work performance reports to stakeholders on a weekly basis. Monitored the project and reviewed reports while taking corrective action, preventive action and following through with defect repair change requests and updates. CL - Closed out each phase on the project while conducting phase-end reviews and submitted phase closeout repots to stakeholders. Also closed out the entire project by presenting a final report and transitioning the deliverable to the customer. OUTCOME: Delivered on time, and on budget. The customer has implemented the solution and the entire project was a success. Be sure to stay clear of any esoteric, obtuse, or hard to understand language. Also steer clear of abbreviations and acronyms as this could throw off the reviewer and you may get an email asking for more details!!  Keep the application to project management and nothing else!  For example I coded and tested as a description is unacceptable!
